Scientists discover molecule that does double duty in stopping asthma attacks

Scientists from Brigham and Women's Hospital are on the brink of the next treatment advancement that may spell relief for the nearly nineteen million adults and seven million children in the United States suffering from asthma. The scientists discovered two new drug targets in the inflammatory response pathway responsible for asthma attacks.

The study will be published on February 27, 2013 in Science Translational Medicine.

Researchers studied the lungs and blood of 22 people with mild and severe asthma. They saw that immune cells called natural killer cells and type 2 innate lymphoid cells played significant roles in airway inflammation in study participants with severe asthma.

Natural killer cells decreased airway inflammation by encouraging programmed cell death in immune cells called eosinophils, whereas type 2 innate lymphoid cells promoted airway inflammation by secreting cell-signaling molecules called interleukin-13.

Both mechanisms were controlled by a molecule called lipoxin A4 which is responsible for resolving inflammation. To achieve this, lipoxin A4 acted in both pro-resolving and anti-inflammatory ways. The researchers saw that lipoxin A4 encouraged natural killer cells to decrease inflammation by facilitating eosinophil cell death. Lipoxin A4 also discouraged type 2 innate lymphoid cells from promoting inflammation by blocking interleukin-13 secretion.

"Stopping airway inflammation is similar to putting out a forest fire," said Bruce Levy, MD, Pulmonary and Critical Care Medicine Division, BWH Department of Internal Medicine, senior study author. "Firefighters tackle forest fires in two ways?dousing the fire with water and clearing away dry brush that could fuel the fire. Lipoxin A4 does just that to resolve inflammation. It is an airway inflammation fighter that performs the double duty of dampening pathways that ignite inflammation while at the same time clearing away cells that fuel inflammation."

In previous studies, Levy and his team discovered that lipoxin A4 production was defective in patients with severe asthma. Together with their new findings, this observation provides researchers and drug manufacturers with a new direction toward boosting lipoxin A4 in severe asthmatics when designing next-generation asthma therapies.

"Most patients with severe asthma have chronic airway inflammation that never fully resolves. This can lead to daily and often disabling symptoms despite available therapies. Our study provides new information on cellular targets that regulate inflammation and will enable the development of lipoxin-based therapeutics to decrease chronic inflammation in asthma and other diseases." said Levy.

COO Artie Minson Could Exit Aol, Patch Issues To Blame?

aol-coo-artie-minsonAol Chief Operating Officer Arthur “Artie” Minson could be departing the company soon, we’re hearing, possibly over how the company has been handling hyperlocal news site Patch. A spokesperson declined to comment for this story, Minson hasn’t returned my call, and our sources say that it’s not a done deal. Bloomberg was the first to report on the news. Minson was promoted to the COO job in June of last year as part of a larger reorganization, moving over from his three-year stint as CFO.?His official bio is basically a big list of positions that he’s held at either Aol or Time Warner (hey, remember Aol-Time Warner?) going back to 2004. In his current position, he’s been responsible for leading the company’s three business units ? the membership group, the brand group (which TechCrunch is a part of) and Aol Networks. One explanation we’ve heard for Minson’s departure is that Aol is trying to boost the numbers for Patch?by lumping in traffic and revenue from the Huffington Post’s local sites. Patch is still unprofitable and?missed its sales target in Aol’s most recent earnings report.?Minson was supposedly unwilling to go along with that plan, which is why he’s leaving. None of this has been officially confirmed. The departure of executives like Brad Garlinghouse from Aol?spurred a concerned investor letter back in 2011. The letter also mentioned the departure of Michael Arrington and Heather Harde from TechCrunch. Then exactly one year ago, it?announced a few more departures, including CTO Alex Gounares. Whatever happens with Minson, other exec changes are also happening. Aol is also looking to bring in a new CEO for the content brands division, according to a report from Kara Swisher?out today.

Fan's NASCAR accident video back on YouTube

When a YouTube video is taken down for copyright infringement, most people get it: Don't post videos that are someone else's legal property. But when YouTube quickly re-posts a video it had pulled, that's an unusual step for the largest video-sharing site in the world.

At NASCAR's request, the Google-owned YouTube removed the 1 minute-and-16-second video taken by a high school student, Tyler Andersen, who was at the the NASCAR Nationwide Series auto race at Daytona Beach Saturday. His video captured part of the horrific accident that injured at least 28 fans, with chunks of debris flying into the stands.

When Andersen posted the video on YouTube, he was clear about his reason for doing so:

No disrespect intended to any of those injured or their families. I was just sharing my experience with a worldwide audience. I will continue to keep all affected by this incident in my prayers and I thank God for protecting me. Thank you.

NASCAR asked Google to take down the video, and it did. The Atlantic Wire points out that "NASCAR's legal fine print on any ticket says they own the rights to any video, sounds or data related to a race. The question became, eventually, whether or not that legal fine print extended to a fan video. Observers criticized NASCAR for taking the video down in the middle of a news story that was still unfolding."

But NASCAR says this wasn't about copyright infringement.

"The fan video of the wreck on the final lap of today's NASCAR Nationwide Series race was blocked on YouTube out of respect for those injured in today's accident," Steve Phelps, NASCAR's senior vice president and chief marketing officer, said on Saturday in a statement shared with NBC News.

"Information on the status of those fans was unclear and the decision was made to err on the side of caution with this very serious incident."

And so YouTube reversed course, allowing the video back up late Saturday, saying in a statement to The Washington Post, and again Monday to NBC News:

Our partners and users do not have the right to take down videos from YouTube unless they contain content which is copyright infringing, which is why we have reinstated the videos.

By late Monday, the video had done well over 600,000 views. NBC News has asked YouTube for comment, and will update this post when we hear back.

Phelps reiterated NASCAR's stance in a statement to NBC News: "This was never a copyright issue. This was never a censorship issue," he said. "The video ... was blocked out of respect for those injured in the accident. Google decided to lift that block."

What does it mean for most of us, who walk around with HD camcorders in our smartphones? Does the fact that Andersen's video continues to survive ? and even thrive ? mean that we can post our own footage of ticketed sporting events? Though tickets tend to warn against such behavior, are we really forbidden from taking a video and posting it on YouTube?

In many cases, yes. If the ticket is a contract, you may be in breach.

YouTube's "decision to allow the video to remain available, while a positive sign in terms of YouTube's willingness to scrutinize claims of copyright infringement, does not in any way prevent NASCAR from pursuing other remedies against the poster of the video ? including, potentially, enforcement of the contract embodied on the ticket or a direct claim of copyright infringement against the poster," Jeffrey P. Hermes, director of the Digital Media Law Project at Harvard's Berkman Center for Internet & Society, told NBC News Monday.

(In this instance, NASCAR has not indicated it would pursue such action.)

However, "NASCAR also cannot claim that the fan has granted NASCAR ownership of that recording based merely on the fine print on the back of a ticket," Hermes said.

Besides, he thinks there's "a serious question as to whether NASCAR has a valid copyright claim in an unscripted sporting event," such as Saturday's race. It's the kind of event, he said, that is "different from a scripted 'performance'" such as a rock concert "in which copyright might arise under U.S. law."

(Translation: Don't even think about posting that Beyonc? concert footage.)

Corynne McSherry, intellectual property director of the Electronic Frontier Foundation, told NBC News Monday that YouTube's decision to re-post the NASCAR video is "the right decision, because NASCAR does not hold the copyright in a fan video."

The EFF has seen this sort of thing before. When Showing Animals Respect and Kindness, an animal-rights activist group, filmed rodeos in order to demonstrate alleged abuse, the Professional Rodeo Cowboys Association fired back, requesting takedown of 13 videos. At the time, YouTube responded by eliminating the activists' account.

When the EFF took the case to court, it was settled in 2009. The agreement protects the group's "right to publicize their critiques."

"The (rodeo association) has no copyright claim in live rodeo events, just as NASCAR has no copyright claim in fan videos," says McSherry.

While the case didn't set a precedent, she said, "the law on this is not ambiguous: absent some other arrangement or exception (such as a work for hire), copyright goes to the person who created the video, not the person who created the event."

ZTE Grand Memo handson a first look at Qualcomm's Snapdragon 800 in action

ZTE caught us by surprise today, outing a Snapdragon 800-infused version of the Grand Memo at its MWC 2013 presser. The device, which still features the same 5.7-inch 720p display, 13-megapixel rear camera and 3,200mAh battery, is nearly indistinguishable from the other variants we've seen. But for this go-round, ZTE's added in Dolby Digital Plus Surround for a superior audio experience and an extra GB of RAM.

Its external chassis may have remained the same -- plasticky and lightweight -- but the difference in this 8.9mm thick Memo's real-world performance is immediately noticeable. OS actions on the skinned Jelly Bean UI don't just zip, they now quite literally fly -- truly, this device is deserving of the long-deceased "beast" moniker. Although international tradeshow WiFi connections are typically dreadful, we did manage to successfully run the Sunspider benchmark to get a quick taste of its rendering performance. The result? The Grand Memo notched a score of 1,186.7ms -- an unimpressive result when contrasted with the Tegra 4's 499ms (as derived from a reference tablet).

As for ZTE's MyFavorite UX layered atop Android 4.1.2, well, we're pleased to say it yields some neat animations. The entire homescreen, including widgets, revolves with a smooth 3D effect that just imbues the handset with a special "feel." It's a shame then that the Memo's encased in a less than premium shell, as it creates a disconnect between the smoking internals and humdrum exterior. But with a newly announced commitment to building its brand (a sentiment popular with Chinese OEMs as of late), this particular Memo is proof positive that ZTE's serious about raising its profile and consumer perception.

You won't be seeing this Grand Memo hit US shores anytime soon -- it's a China and Europe-only affair for now. So while you wait (and hold onto hope), check out our video tour after the break.

Even though Jennifer Lawrence fell on her way up to accept her best actress Oscar for ?Silver Linings Playbook,? perhaps the most embarrassing moment for the 22-year-old came after the show.

While ABC?s George Stephanopoulous was interviewing Lawrence in a post-show wrap-up, three-time Oscar winner Jack Nicholson, 75, came up behind her and said, ?You did such a beautiful job,? before adding ?I would love to interview you.?

Nicholson, who had earlier co-presented the best picture award with Michelle Obama, added to the flirting when he told Lawrence, ?You look like an old girlfriend? to which the actress gamely quipped ?Oh really, do I look like a new girlfriend??

Nicholson didn?t miss a beat. ?I?ve thought about it," he said.

With a smile, the surprised actress exclaimed ?Oh my God!? and buried her head in her hands.?

She really shouldn't have been surprised. On a night where she pulled in the best actress award, flipped off the press room, and nearly fell on her face, getting hit on by someone old enough to be her grandfather was just par for the course.

Obesity Reduces Quality of Life in BoysFor boys, being overweight or obese significantly lowers their quality of life compared to healthy weight peers.? Interestingly, these results were not found in girls.

The study, published in the Journal of Adolescent Health, also showed that quality of life (QOL) scores improved for children of either sex whose weight status changed from overweight/obese to normal.

The research involved more than 2,000 Australian school children who were about 12 years old at the start of the study in 2004-2005. The researchers followed up with the children after five years.?

The participants then answered a questionnaire designed to measure whether being obese (also known as? adiposity) influenced their quality of life at age 17 or 18.

?Adiposity in boys was associated with poorer quality of life during adolescence. This association was not observed among girls.

?In both boys and girls, though, persistent overweight or obesity was related to poorer physical functioning after the five years. In contrast, weight loss was associated with improved quality of life during adolescence,? said Bamini Gopinath, Ph.D., senior research fellow at the University of Sydney in Australia.

The questionnaire measured the children?s physical and psychosocial health.?It also calculated a combined total quality of life score. The psychosocial health summary score reflected how well the teens were functioning emotionally and socially.

The study revealed that both males and females who were obese at the start of the study and who later reduced to a normal weight had far better physical functioning scores than those who remained obese after five years. These physical functioning scores measured one aspect of the overall quality of life score.

?The findings suggest that an unhealthy weight status and excess body fat could negatively impact the mental and physical wellbeing of adolescents, particularly boys,? said Gopinath.

He noted that the study highlights the value of looking at the quality of life among obese teens in both clinical practice and in research studies.?

He also added that ?obesity prevention and treatment efforts [ought] to address the broad spectrum of psychosocial implications of being obese as a teenager.?

Lawrence J. Cheskin, M.D., director of the Johns Hopkins Weight Management Center, noted that the differences in quality of life and physical functioning between obese and normal weight teens has not been carefully done before.

?The fact that QOL improved with improvement in weight over time is also important,? said Cheskin. He added that parents, health care providers and teenagers need to understand the far-reaching effects that being overweight can have on a teen?s enjoyment of life.

If Synaptics has a favorite song, we'd assume it'd be Touch Me, after all, the touchscreen maker does want customers to get handsy with its products. The company is launching a pair of mobile touchscreens here at MWC that we can expect to see laminated to some Gorilla Glass on a bunch of handsets at next year's Spanish smartphone shindig. The ClearPad 3400 is a capacitive touchscreen designed for flagships that offers a greatly improved signal-to-noise ratio for devices of up to six inches.

It's also announcing ClearPad Single-Layer On-Cell (SLOC) Multitouch solution, an interface for budget devices that eliminates the touchscreen sensor and border while still offering five-finger touch. The Pattern: Explanations for reconnecting with an Ex! | The Quarter ...

Going for dinner with the ex (the ex before the most recent ex) was not on the list of things to do this week. I had opted out of going to an expat party in Amsterdam with my friends. I had things to do and I could already predict the party; drunk Mediterranean guys with one goal. So I was looking forward to a quiet night in. Randomness is an understatement when out of the blue an ex contacts me to go to dinner. It took me a few minutes to realise my supply of U.S. Cold medicine had not gone to my head and I didn?t read the message wrong. Reading my blog you are aware that randomness is no stranger to my life, so I thought to just roll with it. Not going to lie, I spent a good hour wondering why the ex who I thought hated me wanted to meet for dinner. I debated standing him up, I refused to change lingerie just in case it was one of those kind of messages. I consulted with friends, but finally I said screw it and why not? I could have had this dinner long ago. Hell, I could have a dinner with all my ex?s present. So dinner turned to a game of pool and pool turned into several cups of tea at home. Whilst I cleaned the tea cups which I successfully did not throw at my ex (as I did 9 months ago). Another random life thought came to me; there seems to be a pattern. Why is it that when we want to form a normal relationship with our most recent ex, it doesn?t work. Yet, a normal relationship with our ex before is suddenly possible? Must there always be an ex in between?

If I had to present a thesis on why I am moving on from my recent ex, then I could. However, despite my attempt at bringing normalcy to our friendship that we say we both want to have, a thesis would be a lost cause to us at present. My recent ex has been avoiding eye contact to the point I might as well wear a pirate patch and he wouldn?t notice. Our ability to talk about everyday things is so absent that I might as well say I am moving to Siberia and see if he notices. Maybe it is sexual tension or maybe it is just too soon, but I am ready to say Oy Vey and down a glass of whisky because of this situation. Why do I want normalcy between us so badly now? Why not? Why must there be uneasiness if you both know your not meant to be with the other? Why not enjoy long talks as friends do? Why not feel there is no one I need to avoid in Holland?

It seems there is an unwritten protocol of when you can become friends with your ex again. I have realised it is when there is a relationship between your relationship with that person. For example, after you break up, you both see other people and possibly form new relationships with other people. Then when you break up with the new relationship it is now okay to contact your previous ex. There is a buffer relationship between you two. It is a safety net. You both proved you moved on from the other. There is time between you. You have had a chance to evaluate relationships in general and more importantly your energy of avoidance is now saved for avoiding an argument with your recent ex. You come to a point where there is no reason, but to not be friendly to the previous ex. Which is what Friday was for me. We both had a breakup buffer between us. We both moved on from the other and found ourselves in other relationships and with a newer breakup than our own.

There was a third wheel present at our reconnect dinner, our most recent ex?s. The only thing is that they have no clue they were present and they were physically invisible to us. They were our buffer of safety, the reason why we could now reconnect. The relationships between our own past relationship has provided time and an opportunity for personal growth and realisations. We can laugh about all the things we had wrong and right. We accept that everything happened for a reason. You feel comfortable around the other. You are at a point where you both just want to bury the hatchet.

Maybe I have it all wrong and we decided to meet just because we were bored and had nothing better to do. However, I have realised this is not the first time in my romantic history of this pattern. I am liking this pattern that gives some explanation for the randomness of what was my Friday night, but more importantly I like that this pattern outlines the importance of time and realisations. Still I have an urge to invite my two favorite (and only ex?s in NL) to my next Shabbos (Friday night) dinner table just to enjoy life. However, my patterns are telling me to take a chill pill and let my recent ex be, reconnect with the one ex and just enjoy waiting for the next prince charming to come along. All I can predict is that one day I will find myself in another random Friday night with another ex and we will be reconnecting because we have an invisible relationship buffer between our own.

Japan?s PM Shinzo Abe may announce Japan?s participation in the Trans-Pacific Partnership free-trade talks to the Diet (the Japanese parliament) on either Feb. 28 or March 1, according to sources.

Abe has faced stiff internal LDP resistance to the TPP and may have to seek exemptions for some agricultural products, and other measures to support local farmers.

Earlier post re the World Trade Organisation calling on Japan to implement structural economic reforms, such as freeing up trade. Entering TPP talks would be progress on this.

HBT: Cardinals' lack of action is their undoing

It?s hard to blame the Cardinals for staying the course. Even with free agents Kyle Lohse and Lance Berkman set to depart, St. Louis ended 2012 with a roster that already looked like a winner entering 2013:

Rotation: Adam Wainwright, Chris Carpenter, Jaime Garcia, Lance Lynn, Jake Westbrook, Shelby Miller, Joe Kelly, Trevor Rosenthal

Bullpen: Jason Motte, Rosenthal, Edward Mujica, Fernando Salas, Mitchell Boggs, Kelly, Marc Rzepczynski, Eduardo Sanchez, Victor Marte

Lineup: CF Jon Jay, RF Carlos Beltran, LF Matt Holliday, 1B Allen Craig, C Yadier Molina, 3B David Freese, SS Rafael Furcal, 2B Matt Carpenter/Daniel Descalso

What did it make sense to do from there? Upgrade at second base? Perhaps, but it was probably the weakest position in free agency this winter. The Cardinals were rumored to have made a run at Marco Scutaro before he re-signed with the Giants.

Upgrade the rotation? Since the Cards seemed committed to Carpenter and Westbrook, that probably would have meant sending Lynn back to the bullpen and there simply weren?t many starters available in free agency that were better bets than Lynn and those few would have been expensive.

Adding to the bench should have been a priority, and the Cardinals did. Unfortunately, they did so with Ty Wigginton, a defensive liability no longer worthy of a roster spot. They also signed Ronny Cedeno as a shortstop fallback rather than trust an encore performance from late-season surprise Pete Kozma.

Besides those two, the only notable newcomer is lefty specialist Randy Choate.

That the Cardinals did so little might have been just fine if not for the events of the last month. Carpenter is expected to miss the season after his nerve problems returned with a?vengeance. Miller, the early favorite to replace him in the rotation, is sidelined with shoulder tightness. And Furcal, the lineup?s biggest question mark, is still having problems with his throwing elbow.

The Cardinals are still fairly well protected in the rotation. Even if they have to dig down deeper as the season progresses, top prospects Carlos Martinez and Michael Wacha could prove ready. Rosenthal should be a big weapon wherever he happens to be deployed.

The team is also very well set in the outfield, what with top prospect Oscar Taveras available to step in if Beltran goes down. The Cards can even put Craig back in a corner and call up power-hitting Matt Adams to play first base if needed.

The middle infield is a big issue, though, particularly in light of Furcal?s ill health. Ideally, Matt Carpenter will prove capable at second base and win the job there. However, if he can?t handle the defensive responsibilities, the Cards could be looking at spending big chunks of the season with the light-hitting Descalso covering one spot and Cedeno, Kozma and Ryan Jackson vying for the other.

At third base, Freese also has an extensive injury history, and while Carpenter is a fine fallback there, he can?t play second if he?s starting at the hot corner.

With spring training games having even yet to start, the Cardinals? depth has been whittled away. If it stops now, the team is still in very good position, particularly since it should have plenty of flexibility to pull off in-season trades. Still, it?s fair to express disappointment that the team didn?t do better than Wigginton and Cedeno for the bench. A Scutaro to start at second or a Kelly Johnson to give added protection would look quite nice right now. The left-handed-hitting Eric Chavez would have made a lot more sense as a backup to two right-handed-hitting corner infielders than Wigginton does. Time will tell if those misfires come back to bite them.

Israeli forces, Palestinians clash throughout West Bank

RAMALLAH, West Bank (Reuters) - Israeli forces clashed with Palestinian protesters throughout the occupied West Bank on Friday, capping a week of violence amid a hunger strike by four Palestinians in Israeli jails.

Tension and anticipation is rising in the West Bank a month before U.S. President Barack Obama is due to visit Jerusalem and Ramallah, though he has announced no concrete plans to revive Israeli-Palestinian peace talks stalled for three years.

From the precincts of Jerusalem's al-Aqsa mosque, both one of Islam's holiest sites and revered by Jews as the site of their Biblical temple, youths threw stones at Israeli police after Friday prayers.

Dozens of Israeli officers briefly entered the politically sensitive compound. Witnesses said officers fired tear gas and threw percussion grenades at the demonstrators as bystanders and elderly worshippers ran for cover.

A police spokesman said no tear gas was fired, but that protesters were throwing firecrackers.

The old city of Hebron, a bitterly contested city in the southern West Bank sown heavily with Israeli settlers, echoed with percussion grenades hurled by Israeli forces at some 1,500 Palestinian protesters.

At a military checkpoint near the northern city of Nablus and outside a military prison in the central West Bank, Israeli forces worked to clear away makeshift roadblocks and fired rubber bullets towards stone-throwing Palestinians.

There were dozens of light injuries from gas inhalation and rubber and aluminum bullets, witnesses said.

Palestinians seek statehood in territories Israel captured in a 1967 war. Peace talks broke down in 2010 over Palestinian objections to Israel expanding settlements on occupied land. Israel has called for resuming the talks without preconditions.

HUNGER STRIKERS IN LIMBO

The status of four hunger-striking Palestinian detainees was in limbo as Israeli civilian courts failed to rule definitively in hearings held for two of them this week, referring their multi-decade sentences back to military courts.

Israel convicted the men of taking part in militant attacks and freed them along with hundreds of other prisoners in a 2011 swap for Gilad Shalit, an Israeli soldier held in Hamas-ruled Gaza for five years, only to re-arrest them soon afterward.

Lawyers and officials representing the men, who were accused by Israel of violating the terms of their release, say their cases are locked in a legal maze and Palestinian officials hope Egyptian mediation could convince Israel to free them.

"Our prisoners ...(on) hunger strike are engaging in a true battle, a battle of glory against the tyrant," said Ismail Haniyeh, Gaza's Hamas prime minister. "No one of us will forget the prisoners. No one would enjoy being with his children at home as long as those heroes continued to suffer in jails."

The hunger strikers have told representatives of an independent Israeli medical group, Physicians for Human Rights, that they are taking water but refusing medicines and nutrients.

There is little exact information on the health of the strikers, whose on-off hunger strikes have ranged from around 80 to over 200 days, as they have repeatedly refused treatment and been denied regular access to independent doctors.

Israel holds around 4,700 Palestinians in its prison on charges ranging from throwing stones to killing Israelis.

Palestinians widely regard them as heroes of their national struggle against Israel and want them all freed.

Columbus mayor pleads for better schools

COLUMBUS, Ohio (AP) - The mayor of Columbus is pleading with the community to help make the public schools better.

Mayor Michael B. Coleman calls is "a fight for the very soul of our city."

The Columbus Dispatch (http://bit.ly/Vzmref ) reports that Coleman's plea came in his annual State of the City address at a high school Thursday. He challenged school officials, parents and business leaders to come together to turn around the beleaguered city district.

Coleman is proposing a complete study of the district's business practices, a system to find better teachers and more pre-school access for children. He asked the business community to help replace failing schools.

The district has been embroiled in a state and federal investigation into attendance data scrubbing, grade changing and allegations of fraud.

Trans Mountain oil pipeline apportioned for March

CALGARY, Alberta, Feb 21 (Reuters) - Kinder Morgan Energy Partners LP said on Thursday that shippers on its routinely overbooked Trans Mountain oil pipeline system between Alberta and the Pacific Coast will be limited to only 33 percent of their hoped-for volumes in March.

Kinder Morgan said capacity on the system, which carries Canadian crude to the Vancouver-area and Washington state refineries, was over-nominated by 67 percent.

Nominations have exceeded capacity since late 2010.

For next month, Kinder Morgan said total accepted nominations for the system are 290,663 barrels per day for the Trans Mountain pipeline, 134,662 bpd for the Puget Sound line and 78,207 bpd for the Westridge Dock.

New injectable hydrogel encourages regeneration and improves functionality after a heart attack

Feb. 20, 2013 ? University of California, San Diego bioengineers have demonstrated in a study in pigs that a new injectable hydrogel can repair damage from heart attacks, help the heart grow new tissue and blood vessels, and get the heart moving closer to how a healthy heart should. The results of the study were published Feb. 20 in Science Translational Medicine and clear the way for clinical trials to begin this year in Europe. The gel is injected through a catheter without requiring surgery or general anesthesia -- a less invasive procedure for patients.?

There are an estimated 785,000 new heart attack cases in the United States each year, with no established treatment for repairing the resulting damage to cardiac tissue. Lead researcher Karen Christman, a professor in the Department of Bioengineering at the UC San Diego Jacobs School of Engineering, said the gel forms a scaffold in damaged areas of the heart, encouraging new cell growth and repair. Because the gel is made from heart tissue taken from pigs, the damaged heart responds positively, creating a harmonious environment for rebuilding, rather than setting off a chain of adverse immune system defenses.

"While more people today are initially surviving heart attacks, many will eventually go into heart failure," said Christman. "Our data show that this hydrogel can increase cardiac muscle and reduce scar tissue in the region damaged by the heart attack, which prevents heart failure. These results suggest this may be a novel minimally invasive therapy to prevent heart failure after a heart attack in humans."

The hydrogel is made from cardiac connective tissue that is stripped of heart muscle cells through a cleansing process, freeze-dried and milled into powder form, and then liquefied into a fluid that can be easily injected into the heart. Once it hits body temperature, the liquid turns into a semi-solid, porous gel that encourages cells to repopulate areas of damaged cardiac tissue and to improve heart function, according to Christman. The material is also biocompatible; animals treated with the hydrogel suffered no adverse affects such as inflammation, lesions or arrhythmic heart beating, according to safety experiments conducted as part of the study. Further tests with human blood samples showed that the gel had no affect on the blood's clotting ability, which underscores the biocompatibility of the treatment for use in humans.

San Diego-based startup, Ventrix, Inc., which Christman co-founded, has licensed the technology for development and commercialization. Christman also serves on the company's board. "We are excited and encouraged by the results of the study leading to a novel regenerative medicine solution for cardiac repair. The technology offers the potential for a longer and better quality of life for millions of heart attack sufferers," said Adam Kinsey, the CEO of Ventrix.

Veterans diagnosed with reflux issues often start on high doses of drugs, rarely stop therapy

CHICAGO --- U.S. veterans diagnosed with gastroesophageal reflux disease (GERD) are frequently prescribed doses of proton pump inhibitors (PPIs), such as omeprazole (commonly known by brand names such as Prilosec), that are much higher than recommended --- and they are kept on the drug far too long, according to a new Northwestern Medicine study.

PPIs are among the most widely used drugs in the nation, resulting in more than $11 billion in annual direct health care costs in the U.S. PPI overuse has been documented in previous studies, but this is the first study to examine initial prescriptions given to veterans with a new GERD diagnosis.

"We should always have a reevaluation after an initial prescription and ask, 'Does this patient need to be on this medication?'" said Andrew Gawron, M.D., first author of the study and a fellow in the division of gastroenterology and the Center for Healthcare Studies at Northwestern University Feinberg School of Medicine. "Proton pump inhibitors are provided ubiquitously in medicine, and although they provide relief for many patients, optimal prescribing is important to avoid prolonged, unnecessary use and cost."

The researchers evaluated more than 1,600 Veterans at the Hines VA from 2003 to 2009. Here are the major findings, which highlight the potential problems:

  • The majority of patients received more than a three-month initial supply of medication.
  • Almost 25 percent of patients were given high total daily dose prescriptions.
  • Very few patients who started on high dose therapy had reductions in dosing more than two years after their initial prescription.

It is recommended that PPIs be prescribed at the lowest effective dose for four to eight weeks to treat GERD. If symptoms persist after eight weeks, efforts should be made to evaluate other potential causes of symptoms and alternative approaches to therapy. This approach is a top priority in the "Choosing Wisely" campaign initiated last year by the American Board of Internal Medicine and the American Gastroenterology Association.

"It seems that once these veterans are prescribed a PPI, they are rarely taken off of it," Gawron said. "Two years after their initial prescription, most are still on the drug."

This study highlights the importance of ensuring appropriate use of PPIs in all patients after they are initially prescribed, Gawron said.

Veterans diagnosed with reflux issues often start on high doses of drugs, rarely stop therapy

CHICAGO --- U.S. veterans diagnosed with gastroesophageal reflux disease (GERD) are frequently prescribed doses of proton pump inhibitors (PPIs), such as omeprazole (commonly known by brand names such as Prilosec), that are much higher than recommended --- and they are kept on the drug far too long, according to a new Northwestern Medicine study.

PPIs are among the most widely used drugs in the nation, resulting in more than $11 billion in annual direct health care costs in the U.S. PPI overuse has been documented in previous studies, but this is the first study to examine initial prescriptions given to veterans with a new GERD diagnosis.

"We should always have a reevaluation after an initial prescription and ask, 'Does this patient need to be on this medication?'" said Andrew Gawron, M.D., first author of the study and a fellow in the division of gastroenterology and the Center for Healthcare Studies at Northwestern University Feinberg School of Medicine. "Proton pump inhibitors are provided ubiquitously in medicine, and although they provide relief for many patients, optimal prescribing is important to avoid prolonged, unnecessary use and cost."

Trattoria Ermes, Modena ? Simon Says

We?d arrived in Modena (a two and a half hour drive from Genoa) just before lunchtime. We?d read in the guidebook about a couple of traditional trattorie which sounded appealing, and when we asked at the hotel they recommended ?Ermes?, and phoned on our behalf to make a reservation, as this was the kind of place which sold out fast, and where people queued outside for the chance of a table.

Our place secured, we set off immediately and five minutes later made our way into a tiny, packed room off a nondescript street. Our eponymous host placed us on a table next to another young couple (her: Tuscan, him: Modenese) and quickly and rather brusquely informed us of the dishes available. This being a home cooking kind of place the options were few and simple. You pay a flat fee of 20 Euro regardless of what you eat, but you have the right to three courses, water, bread, wine and coffee.

I didn?t want to interrupt the experience too much (and was maybe a little intimidated by the close quarters and proximity of the other diners) so I only took a few iPhone photos of the dishes. I went for the maccheroni; oven baked pasta with a very crunchy dark crust on top. Good, moreish comfort food for a cold February lunch time.

For the main course I had rabbit with boiled potatoes. Nothing groundbreaking, and rather starkly presented, but the meat was tender and well seasoned and tasty.

We finished with a kind of light, sweet pastry which is traditional at Carnival time and goes variously by the name of bugie, frappe, or chiacchere among other regional variants.

So, the food was simple, honest, tasty fare. Nothing spectacular, but popular with those who like traditional regional specialities prepared in the same way their grandmothers used to do it. But perhaps what made it a more memorable experience was Ermes himself. He?s quite the local celebrity and he probably exaggerates his moods swings and outsized personality at least a little in order to keep his clientele entertained. A few examples: at one point his elderly aunt stopped by for a bite to eat. He dumped her at the end of a large table full of strangers and they bickered with each other sporadically throughout the meal. During our main course someone on the table behind ours wanted to know what a particular dish was like, so Ermes picked up my wife?s plate, as she was in the middle of raising a forkful to her mouth, held it out in front of the other diners for them to glance at, then plonked it back on our table.

On the wall there?s a chart indicating his mood that day. The text at tope left says ?For those of you who drink to forget, please pay in advance?.

This was undoubtedly good for a few laughs, and the warm atmosphere and conversation we shared with the other couple on our table meant that the social aspect of the meal was at least as important and enjoyable as the actual food. There are a lot of press clippings on the walls and I wondered whether Ermes? celebrity was overshadowing the restaurant itself. Had it turned into a magnet for the tourist horde (of which, of course, we ourselves were a part) who wanted a caricatured Italian shouting and gesturing and flinging plates of hot pasta around?

But on the other hand we were the only non-locals there that day, so I don?t think his popularity can be put down to catering to foreigners? expectations. The ?character? of the place is certainly a little self-conscious, maybe even a little exaggerated and theatrical, but no less genuine for that. And I did enjoy the food. Of the reviews on sites like Trip Advisor the negative ones expressed mainly disappointment with the food itself, which they felt didn?t match the restaurant?s reputation, and the flat 20 Euro fee which applied even if all they ate was a plate of pasta. But if you?re in the mood for something simple and tasty and filling, and don?t mind a bit of noise and bustle and shouting, it?s a bargain.

And by the time we left a queue was starting to form outside?
